The 3-level classification of parkinsonism is based on the number of parkinsonian signs present. A trained nurse clinician administers a 26-item modified version of the motor portion of the United Parkinson's Disease Rating Scale (mUPDRS) to calculate the presence of 4 parkinsonian signs: bradykinesia, gait, rigidity, and tremor.
