Self-reported history of head injury with loss of consciousness at baseline

History of head injury with loss of consciousness at baseline is self-reported based on two questions.

Q1: Have you EVER had a head injury? 1 = Yes (then branch to next question) 2 = No

IF YES then:

Q2: Have you EVER lost consciousness because of a head injury? 1 = Yes 2 = Suspect or possible 3 = No

1	History of head injury with loss of consciousness - Reported at baseline
